Skarin, MD, FACP, FCCP, earned his MD from the State University of New York at Buffalo in 1961, and received postgraduate training in internal medicine at the Millard Fillmore Teaching Hospital in Buffalo. After a fellowship in hematology and oncology at Boston City Hospital, he joined Brigham and Women's Hospital and Dana-Farber Cancer Institute (DFCI) in 1970. 
He served as Clinical Director of the DFCI Hematology Laboratory as well as Chairman of the Laboratory Committee from 1970 to 1998. He established the multimodality thoracic oncology clinic in 1975 and has served as Medical Director since then. During that time he also was chair of the human protection committee (IRB) for 12 years and became Associate Professor of Medicine at Harvard Medical School. 
He has been the Principal Investigator (PI) as well as site PI on numerous clinical research Phase I, II and III trials, both at DFCI Partners Cancer Care and the Cancer and Leukemia Group B (CALGB). He was appointed a Research Subject Advocate (RSA) in 2002 for NIH funded studies at the Brigham and Women's Hospital General Clinical Research Center (GCRC) and DFCI. 
Dr. Skarin is a board member of the American Journal of Oncology Review, the Journal of Cinical Oncology, Hospital Physician and also president of the Massachusetts Society of Clinical Oncologists. He has published 150 original articles, 97 abstracts, 67 chapters in books and 4 books.

There has been enormous progress in nuclear medicine in recent years, and the impact of this progress has been particularly noticeable in oncology. Research into molecular imaging has led to the development of radiopharmaceuticals that can explore the cellular metabolism, and visualize at molecular and subcellular levels the pathological processes specific to cancer. Equipment development has produced high-technology instruments such as those used in positron emission tomography (PET), able to produce high-quality images that have become indispensable in the diagnostic work-up of cancer patients, because they often reveal alterations and lesions not demonstrated by conventional morphologically oriented techniques such as X-ray imaging, ultrasound, computed tomography (CT), or magnetic resonance imaging (MRI).     Intracranial Metastases: Current Management Strategies

    The current incidence of tumors metastasizing to the brain is high, and brain metastases are now the most common type of intracranial tumor. Current estimates indicate that, in the USA alone, more than 200,000 patients per year are developing brain metastases. Treatment of patients who have these intracranial metastases involves many different medical specialties. Therefore, there is a need for a better integration of these specialties in the orchestration of patient care.
    This volume represents the first up-to-date comprehensive textbook devoted to the subject of human brain metastases in more than 20 years (since 1980).
    There are four main areas of emphasis in the present work:
    1 It provides an overview of the topic of brain metastases: their biology, diagnosis, and treatment throughout history.
    2 Instead of considering brain metastases that arise from tumors in different tissues collectively in a single chapter, each tumor type that metastasizes to the brain is presented in a separate chapter written by an expert on the subject.
    Special categories not previously covered have been included, such as the epidemiology of brain metastases and the quality of life in patients who have them, tumor metastasis to the skull base, and strategies that may help prevent metastasis of cancer to the brain.

    With such advances in neurosurgical techniques since the mid-1990s, even patients who had traditionally been excluded as surgical candidates, such as some of those with multiple or recurrent brain metastases, have been shown to survive longer after surgical treatment. Nevertheless, in the future, more work on the treatment of brain metastases needs to take place. More prospective randomized studies are required on issues such as:
    1 The relative roles of surgery and stereotactic radiosurgery in the treatment of brain metastases. One should note that data on radiosurgery for treatment of brain metastases were not available in 1980 when the last text focusing on brain metastasis was published.
    2 Determination of the best balance between treatment of intracranial metastases with whole-brain radiation therapy and the cognitive outcome and quality of life of the patient.
    A completely understudied area that may hold promise is the role of molecularly targeted therapy for metastatic brain tumors. Scientists in the neurosur gical oncology molecular biology laboratories are working hard to translate their discoveries into therapeutic agents for such clinical use.
    This book will provide valuable insight into the nature and current treatment of brain metastases to all those in the many medical specialties and subspecialties who are likely to encounter these tumors.     Cancer is the second most common cause of death in Americans (see www.cdc.gov). Colorectal cancer kills more Americans than any other malignancy except for lung cancer. Taken as a group, the five most common GI malignancies account for more cancers and more cancer deaths than for any other site.
     
    Flexible endoscopy has given physicians unprecedented access to the GI tract. The ability to endoscopically visualize, biopsy, and apply therapy has had implications for the management of all the major GI malignancies. Accepted applications of endoscopy range from detection of malignant and premalignant lesions (e.g., colonoscopy for colon cancer screening), prevention of cancers through removal of precursor lesions (e.g., polypectomy), surveillance of premalignant conditions (e.g., Barrett’s esophagus), palliation of symptoms (e.g., placement of stents for biliary or esophageal obstruction) or staging of cancers to allow stage directed therapy (e.g., endoscopic ultrasound), and, in selected circumstances, definitive therapy for early stage neoplasms (e.g., endoscopic mucosal resection). This partial list of applications demonstrates the central role that endoscopy plays in management for those at risk for or with a GI malignancy. The wide variety of endoscopic techniques applied suggests a new subspecialty of endoscopy: “endoscopic oncology.” This is similar to “surgical oncology,” as it concerns itself with the subset of endoscopic procedures directly applied for the management of neoplastic and precancerous conditions.

    It becomes apparent that a substantial proportion of endoscopies are performed for a cancer-related indication. To determine what proportion of endoscopic procedures are done out of a concern for cancer or a premalignant condition, a large national database of endoscopic reports (Clinical Outcomes Research Initiative [CORI]) was queried. Indications related to cancer were defined by convening an expert panel.

    The results demonstrated that the majority of endoscopic procedures (63.5%) in these practices were performed owing to a primary concern for cancer. In fact, only for EGD were the majority not done for a cancerrelated indication (32.7%). The great majority of colonoscopy (84.4%), ERCP (59.9%), and EUS (98.7%) procedures are done for cancer-related indications. For colonoscopy, the major cancer-related indications are surveillance of patients with prior polyps (21.3% of cancer-related indications), evaluation of hematochezia (26.2%), follow-up of a positive hemoccult test (15.6%), or surveillance in a patient with a family history of colorectal cancer (17.8%). For EGD, dysphagia was the most common cancer-related indication (62.4%) followed by anemia (23%) and Barrett’s screening/surveillance (12.2%). For ERCP, 98% of the cancer-related indications are related to bile duct obstruction. For EUS, the primary indications related to cancer are FNA of a mass (26%), stage a known cancer (23%), or evaluate a pancreas lesion (23%). With nearly two-thirds of all endoscopic procedures being done out of a primary concern for cancer, it is apparent that all of us who do endoscopy are endoscopic oncologists!
    This textbook examines the interface between endoscopy and oncology. It is organized anatomically: esophagus, stomach, colorectum, and pancreaticobiliary. For each site, the relevant cancers and premalignant conditions are addressed and the use of endoscopy in their diagnosis, management, and treatment discussed in detail. Additionally, the reader will find chapters summarizing the state of the art for nonendoscopic medical and surgical cancer treatment.

    About the Author
    My name is Richard Smith and I am a Consultant Gynaecological Surgeon who specializes in treating women with cancer. I was born and educated in Scotland, graduating from Glasgow University (MB, ChB). I subsequently underwent postgraduate training in the West of Scotland and London. In the late 1980s, I worked at St. Mary’s Hospital in London and did my thesis on the interaction of infections with cervical cancer (MD, Glasgow).
    I have worked for many years the Chelsea and Westminster Hospital and have recently joined the new West London Gynaecological Centre at the Hammersmith Hospital in West London. In terms of Teaching, I am a Senior Lecturer at Imperial College School of Medicine, London, UK, and Adjunct Associate Professor at New York University School of Medicine.
    I have had a long running interest in doctor–patient communication and have edited two series of books (The “Guide to …” series and another series, “Patient Pictures”), which were designed to help doctors and nurses explain surgical operations to their patients. These books have sold well (approximately 240,000 copies), suggesting that colleagues, both nursing and medical, are very much interested in this area, not what you would think from looking in the press!
    I have always been keen on the synthesis of “standard” or orthodox medicine with “complementary” medicine. This started with being taught hypnosis, practicing it, and has subsequently widened out to include referring my patients for hypnotherapy, homeopathy, acupuncture, counseling, and psychotherapy. Over the last decade I have also become convinced that although I believe that people with some form of religious belief do not survive longer, I am sure they cope better with their cancer. For those with no religious sentiment there are other ways of helping them to find the inner self. The synthesis of these factors allows for a truly holistic approach.
    My co-author, Giuseppe Del Priore is also a gynecologist specializing in cancer treatment. He works at New York Downtown Hospital, Weill-Cornell School of Medicine in New York City and has a long-standing interest in doctor–patient communication. He has been a pioneer in the field of fertility sparing surgery.
    The artist for the book is Ms. Dee MacLean, who is one of the foremost medical artists in the world today. She has an outstanding ability to make complex anatomy look comprehensible to both doctors and patients and has a long running interest in illustrating medical problems for patients. The Rev.     Nutritional oncology is an interdisciplinary field that bridges two areas of metabolic homeostasis—nutrition and cancer.     HPV and Other Infectious Agents in Cancer: Opportunities for Prevention and Public Health

    The role of viruses in the etiology of cancer has been a focus of scientific inquiry for much of the twentieth century; the more recent development of vaccines against the hepatitis B virus and some types of human papillomavirus has provided an important public health opportunity for preventing the development of liver and cervical cancers, respectively.
    A fuller appreciation of this topic is a priority in cancer control. Thus, the National Infectious Agents Committee of the Canadian Partnership Against Cancer’s Primary Prevention Action Group, in partnership with Canadian Cancer Society, sponsored the development and publication of this volume.
    Despite signifi cant improvements in cancer prevention, treatment, and survival, more and more Canadians are diagnosed with cancer. This story is repeated in many jurisdictions in the world. One driver of such trends is an aging population, but cancer is not only a disease of the elderly. For example, cancer is the leading cause of death in middleaged adults in Canada. In the 35- to 64-year age group, cancer causes more deaths than heart disease, stroke, injury, and infectious diseases combined.
